In my opinion the biggest mistakes, during a development cycle, are done at the very fist stage.

This is a well known fact.Smile | :)
But it seem no one hase the time and money to work that way. The whole fuss about 'agile processes' and so on seems to mainly stem from the fact that there needs to be a continual flow of small success reports: Management, marketing and customers want to see something early on, just to say "We want it different."
Proably, the human mind is working that way.

We have to define the requirements for them with a few information what they really want.

And I am sure they are not content with what they see. But I think this is normal. In my part of the industry (bioinformatics), we often have to assist academeic customers by researching some new applications. There is way any one could have guessed beforehand what he would need most (short of saying "We need anything!"). We simply need to code, show and improve.
And while we are at it, marketing does its work and sells our first shot Mad |

"We trained hard, but it seemed that every time we were beginning to form up into teams we would be reorganised. I was to learn later in life that we tend to meet any new situation by reorganising: and a wonderful method it can be for creating the illusion of progress, while producing confusion, inefficiency and demoralisation."
