There is an actual reference in the web.config that links to the report server version you are using. You just need to ensure this is updated to the correct version and shows it. Mine still displayed to the old version. (Version & PublicKeyToken)

I am not sure I understand correctly...
CSV is Comma-separated values... Thus there is no formatting required at all. If you are talking about formatting, don't you mean to ask how to wrap column header in excel?

Did you publish it to the service? If it shows your PC name chances are that you published to your pc and never the site itself. Thus your PC is the router to the site instead of the site itself in a way
