There are many PDF generation controls available. Have a search in google to find some.

A free approach is to use iTextSharp (open source and .net implemented)
You can use third parties libraries (tall pdf, ...).

Use a Dictionary then, and copy the items you are interested in over to it, then bind to the dictionary. This is going to be a relatively slow process (depending on the number of rows present). Your logic will look something like this:
Dictionary<int, string> dictionary = new Dictionary<int, string>();
foreach (.... in ...)
{
int item = field1;
string data = field2;
if (!dictionary.ContainsKey(item))
{
dictionary.Add(item, data);
}
}
myGrid.DataSource = dictionary;
myGrid.DataBind();

You're basically asking us to do all the work for you, which nobody will. Take it step by step. First, the upload dialog. Goggle about it. Read the tutorials you find, implement it and then, if you have any problems ask for our help.
You're wasting your time trying to get us to do your work. And so would we by giving you the code. The road to knowledge begins with learning.

You tell us? It's your code - so presumably you've tried it. What do you think the problem is?
BTW - you really need to either check the ignore HTML checkbox or use < so that you post the actual code and not a mangled pile of garbage.
