Yes, you need help on many aspects. First, pick a better subject line, instead of "I NEED HELP?". Secondly, clean up the grammar a bit. Thirdly, be a bit more clear about what you are having trouble with. Just saying the program is slow because of XML data doesn't help diagnose your program's problem very well.

Oh come on, calling SendMessage after looking up the message in MSDN is not rocket science.
The message definition can be found in CommCtrl.h in the Platform SDK Includes: TV_FIRST is 0x1100 and TVM_SETINDENT is TV_FIRST + 7 .

Just try it, it's really not that hard!

You have to get familiar with the DICOM spec. It's not easy. I incorporated DICOM images and data into a system I wrote a couple of years ago. Your best bet is to check out the openDicom project written in C# and go from there. There's more to it than I could try to answer in a post. Actually, you just gave me an idea for an article!

Oh, I didn't know this existed, we use MyDICOM for the DICOM files, and the Atalasoft DICOM library to read the various image compressions.

It's a nice library. I wrote a radiology system a couple of years back and needed dicom support. Creating a viewer was a breeze. I used it because it's written in C# and that made it a whole lot easier to get into the internals. I don't have anything to compare it to, but you should check it out.
