Thanks, anyway, for the non-answer, but what I mean by "fully functional" is that the pdf in question has form fields and checkboxes that are properly named and whose properties are properly set up to return the values I want to use once submitted. It's not my choice as to whether or not to use a PDF. Clients can be particular. The pdf will reside on the host web server, but my client wants to have a submit button incorporated on it so that the user doesn't have to print it out and fax it to him after filling it in. He wants the data delivered to him in an e-mail. I agree that it would be infinitely simpler to create a user form with equivalent checkboxes and form fields on an aspx or php page, but alas, as I said - it's not my choice.

Plus - I'm always game for learning something new. I love a challenge. Need a little help, that's all. Smile | :)
