Rating a form based on user selection

Hai Friends,
How to give rating to my form ?
Example: Many real estate agent are there. When user selects one agent, I want to show rating for that agent.
I need help on implementing this feature.

So, what's the problem? And how is that related to forms? Basically, you would need to use Ajax, which does not require a form.
