Video Upload and display in page in PHP and MYSQL

I am working with a code which upload a video file and save to folder and in database simultaneously. But now problem is that how to display a video on a page by matching the name of video that save in database with name of video save in folder.......Also how to create a thumbnail of particular video with duration of that video....If possible then provide me the code also....Please give me the solution of this problem..Thnxs
